Hertfordshire County Council, Public Health & Wellbeing Service Research

Hertfordshire Public Health Service, within the County Council, is researching the need for a new integrated Health and Wellbeing Service for Hertfordshire. They would like to know what residents' current health and wellbeing needs are, and how they would like to receive advice, support and services to help maintain and improve their health.

They would be very grateful for your views as a Hertfordshire resident to help in the development of this service.

The survey questionnaire should take about 15 minutes to complete, and as a thank you they can enter you into a prize draw for £200 of Shopping Vouchers (e.g. 'Love to Shop'). (Prize to be drawn on 31 March 2021.) All your responses will be treated in the strictest confidence and will only be used for the purpose of developing public health services within Hertfordshire.
